Evidence of Citizenship – Original Birth Certificate
Both Parents and the applicant needs to be present in person: Parents must have their original Passport/ State ID/ Driver’s Licenses Present.
- If a parent is not able to come in then they can complete DS-3053 (Click here to fill out) and ensure you consult us if the application is correctly filled out before getting it notarized.
Passport Photographs: We do offer passport photos with a cost of $20 if you’d like to opt for it.
US Government Fees:
- For Minor Under the age of 16
  - Passport Book: $160 or Passport Book & Card: $195.
  - Passport Execution Fees: $35 [Payable at the post office during the sealing of application process.
- For Minor above the age of 16.
  - Passport Book: $190 or Passport Book & Card: $225
  - Passport Execution Fees: $35 Payable at the post office during the sealing of application process.

Model of paymnet Accepted:

Your signature must match the one on your ID or previous passport.
If your signature has changed, let us know—we can provide a letter of explanation.
We do not guarantee approval, as passports are issued by the U.S. Department of State.
We do guarantee on-time delivery if your application is approved.
There is an ongoing investigation (city, state, or federal level) under your name.
You owe taxes or child support.
The Dept. of State cannot verify your identity.
No refunds are issued unless there's a delay on our end in submitting the application (this has never happened).
Delays caused by the agency or applicant's background do not qualify for a refund.
