01. Visa application form. Fully completed, signed and notarized original application form.
02. Original passport. The passport must be valid at least 6 months after the intended date of departure from the territory of the destination country and has at least 2 blank visa pages.
03. Passport-type photograph. Include 2 passport style photos, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a charge associated with this service.
04. Proof of status. A copy of immigration status in the United States such as a resident card, work, or study permit. Best SAME DAY PASSPORT &VISA SERVICES NYC cannot assist US B1/B2 visa holders at this time.
05. Itinerary. A copy of round trip tickets or detailed itinerary for the duration of the trip. The applicant's name must be on itinerary.
06. Doctor's note.. Letter from physician confirming that the applicant is free of all communicable diseases.
07. Tourist voucher. If applicable, a letter from the travel agency confirming the purpose of travel.
08. Personal Letter. A personal letter from the applicant addressed to the Embassy of Afghanistan, explaining the purpose of the trip, dates of travel, cities to be visited, and place of accommodation.
