DS-11 Passport Application (Click here to complete the passport application).
Gather Evidence of US Citizenship.
Driver’s License or State ID
Passport Photographs – We do offer passport photos with a cost of $20, if you’d like to opt for it.
US Government Fees:
- Passport Book: $190 or Passport Book and Card: $220
- Passport Execution Fees: $35 [Payable at Post office during the application sealing process]

Mode of Payment Accepted:

Your signature must match the one on your ID or previous passport.
If your signature has changed, let us know—we can provide a letter of explanation.
We do not guarantee approval, as passports are issued by the U.S. Department of State.
We do guarantee on-time delivery if your application is approved.
There is an ongoing investigation (city, state, or federal level) under your name.
You owe taxes or child support.
The Dept. of State cannot verify your identity.
No refunds are issued unless there's a delay on our end in submitting the application (this has never happened).
Delays caused by the agency or applicant's background do not qualify for a refund.
