01. Visa application form. 02. Original passport. The passport must be valid at least 6 months after the intended date of departure from the territory of the destination country and has at least 2 blank visa pages.
03. Passport-type photograph. Include a passport style photo, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a surcharge associated with this service. Applicant must not wear glasses in their photographs.
04. Driver's license. Copy of driver's license or state issued ID, or ORIGINAL major utility bill (Water, Gas, Electric, Sewage) for the most recent month, showing the applicant's name and current address. The address must not contain a PO Box. Address MUST match home address in your applicant profile.

03. Conference Clearance. If the Conference/Seminar/workshop is organized by a private entity, then a CONFERENCE clearance from Ministry of Home Affairs and Ministry of External Affairs is required.
04. Conference invitation. If attending, a letter of invitation from the organization or conference hosting the event.

03. Proof of residency. Proof of address must match with the present address as per application: (any ONE of the below documents) - State Issued ID OR - Driving license OR - Utility Bill - Water (should be recent, no more than 3 months old) OR - Utility Bill - Gas (should be recent, no more than 3 months old) OR - Utility Bill - Electricity (should be recent, no more than 3 months old) OR - Valid Lease Agreement OR - Mortgage OR - Income Tax return OR - For College/University Students living on campus in University housing/dormitories, a letter from your institution on the institution letterhead explaining the exact address that you presently reside in will suffice. - Marriage Certificate in case the address in spouse’s name, birth certificate of child in case of minor applicant along with parent’s address proof. - In case of applicant residing with parents/children/ relatives, a notarized undertaking from them with one of the copy of address proof as indicated above. - Hotel bill - If the customer on L1/L3 staying in a hotel for a project for the past couple of months and don’t have the address proof or state ID. NOTE: DO NOT CUT YOUR DRIVER'S LICENCE COPY DOWN TO SMALLER SIZE Important: PO BOX number is NOT ACCEPTABLE as valid Proof of address.
04. Additional particular form (Annexure - A). the form is mandatory to be filled by applicant (even for minors); application will be kept on hold if this form is not submitted.

09. Reccomendation letter. Recommendation letter from Media Company in USA indicating visiting place, purpose and duration - original.
